Please give this form to your clinician or treating physician to complete. The form can be returned to the LAP office on your home campus, or emailed to lap@hancockcolleg.edu 

Classroom ASL Interpreter/Transcriber Request You must have received approval for this accommodation from your LAP Specialist or LAP Counselor before completing this form.

If LAP is requesting information on your behalf, you will need to complete and sign a Release of Information form. Please remember that some medical offices will require you to pick up or receive documentation.
