Student Forms

To complete a form, you may be prompted to sign-in to your Hancock portal.  If you have any questions, please see a counselor before submitting a form.  

This process allows substandard grades to be disregarded in the computation of your cumulative GPA if the work was not reflective of your current scholastic level of performance.  Alleviated work will always remain on your official transcript, and it shall be annotated in a manner that all work remains legible ensuring a true and complete academic history.  Only the units and grade points will be alleviated. 

Academic Renewal Options:

  • Option 1- Alleviate the entire semester when semester GPA is below a 2.0.
  • Option 2- Alleviate individual substandard grades (D or F) within the semester.

Eligibility:

  • A maximum of three semesters can be alleviated.
  • A period of one year has elapsed since the work to be alleviated was completed.
  • A student must have completed, since the work to be alleviated, at AHC or another accredited college/university:
    • 18 semester units with at least a 2.4 GPA, or
    • 24 semester units with at least a 2.0 GPA 

To file an academic renewal, you are required to meet with a counselor.  A counselor will determine your eligibility and file the form on your behalf.  If you attended another college, all official transcripts must be submitted to the Admissions and Records Office before meeting with a counselor.

This form is used to request excess units only if you are in good academic standing.  Do not complete this form if you are on academic/progress probation and wish to enroll in excess units.  You are required to meet with a counselor.  

  • 20.5+ units for either fall/spring semester
  • 12+ units for either summer/winter semester

Excess Unit Request

If you are unable to access the form via your Hancock portal, use the following link to complete the form.  You will be prompted to create a dynamic forms account. 

This form is used to request a fourth course attempt.  This means you have taken the same class three times and wish enroll for a fourth attempt. 

This form has a deadline.  Verify deadline before you submit. No late forms will be processed. 

Excessive Course Attempt Appeal

If you are unable to access the form via your Hancock portal, use the following link to complete the form.  You will be prompted to create a dynamic forms account. 

This form is used to request high school foreign language competency credit for successfully completing 2-years of the same foreign language.

Foreign Language Competency

If you are unable to access the form via your Hancock portal, use the following link to complete the form.  You will be prompted to create a dynamic forms account. 

This form is used if you are on academic/progress probation and wish to exceed your unit limit.  

To file an plan of action, you are required to meet with a counselor.  A counselor will determine the conditions of your enrollment. 

If you are currently enrolled in the prerequisite course at another college, file a "Prerequisite Provisional" for temporary prerequisite clearance.  Do not file a prerequisite appeal.  

If you completed the prerequisite course with a grade of C or better at another college, you are required to submit an official transcript to the Admissions and Records Office for prerequisite clearance.  Do not file a prerequisite appeal.   

This form is used to appeal a prerequisite course despite not completing the required course at an accredited college.

Prerequisite Appeal

If you are unable to access the form via your Hancock portal, use the following link to complete the form.  You will be prompted to create a dynamic forms account. 

This form is used to appeal CHEM 120-Intro to Chemistry and advance to CHEM 150- General Chemistry 1 based on successfully completing either one of the following:

  • High School Chemistry with an A grade & Algebra 2 with a B grade, or
  • AP Chemistry & Algebra 2 with a B grade.

We strongly encourage you speak to a counselor to ensure you are ready to advance to CHEM 150.

Prerequisite Appeal for CHEM 150

If you are unable to access the form via your Hancock portal, use the following link to complete the form.  You will be prompted to create a dynamic forms account. 

If you completed the prerequisite course with a grade of C or better at another college, you are required to submit an official transcript to the Admissions and Records Office for prerequisite clearance.  Do not file a prerequisite provisional.   

This form is used if you are currently enrolled in the prerequisite course elsewhere.

I am currently enrolled in the prerequisite course at another college:

If you are currently enrolled in the prerequisite course at another college, you are able to submit a prerequisite provisional, which will temporarily lift the registration block pending an official transcript showing a grade of C or better.  You will be required to provide an official transcript to the Admissions and Records Office by the designated deadline listed on the form.  Failure to submit an official transcript will result in a drop.   

I will be taking the AP exam in May:

If you are in the process of taking an AP exam in May and wish to enroll in the advanced course for the fall semester, you are able to submit a prerequisite provisional, which will temporarily lift the registration block pending an official transcript from the College Board showing a score of 3, 4, or 5.  You will be required to provide an official transcript to the Admissions and Records Office by the designated deadline listed on the form.  Failure to submit an official transcript will result in a drop.

Prerequisite Provisional

If you are unable to access the form via your Hancock portal, use the following link to complete the form.  You will be prompted to create a dynamic forms account. 

This form is used to get reinstated as a dismissed student after you have been absent for one primary semester.  You must provide reasonable assurance you are ready to succeed.  If approved, you will be required to follow the conditions of your reinstatement. 

To file a reinstatement contract, you are required to meet with a counselor.  A counselor will determine the conditions of your enrollment.  

This form is used by students on academic/progress probation to request an instructor to submit a mid-semester grade.
